About the role
Western NSW Primary Health Network (WNSW PHN) is currently seeking a motivated individual who has a passion for primary healthcare and community wellbeing.
The Project Officer Care Finder position supports the strategic objectives of Western NSW Primary Health Network (WNSWPHN) to lead, support and strengthen person centred primary health care, by developing and coordinating activities related to older people’s health, aged care and palliative care.
The role will focus on the management, needs assessment, commissioning, contract management, implementation, and coordination of WNSW PHN care finder program. The program will strengthen support for care finders, including access to local and regional communities of practice to build their knowledge and skills. The goal of this role is to embed care finders as an integrated part of an integrated local aged care system. This will also involve collecting data and information to support both continuous quality improvement (CQI) initiatives and evaluation of the care finder program.
The position will provide contract management and project support to manage and maintain a network of care finders across the WNSW PHN region. Care finders provide assistance to people within the care finder target population to understand and access aged care and connect with relevant supports in the community. This role will undertake other aged care projects and tasks as determined by organisational need.
The position can be based in any one of our offices, including: Dubbo, Orange, Bathurst or Broken Hill.
The salary for this position is Level 3, Grade 4 ($101,324 + superannuation).

Selection Criteria
Essential
-
Tertiary qualifications in a health-related field and/or relevant experience.
-
Experience managing the implementation of community-based health programs and projects.
-
Demonstrated ability to work collaboratively and build rapport with a wide range of individuals and organisations, within a multidisciplinary team.
-
Demonstrated ability to translate change initiatives into practical strategies.
-
Excellent interpersonal skills, with well-developed written communication skills including writing project reports and management briefings and development of project materials.

Demonstrated understanding of Aboriginal culture including a commitment to cultural awareness and safety.
-
Demonstrated high-level interpersonal and communication skills and high-level competence in using information communication technology.

Demonstrated organisation and time management skills, with proven ability to deliver high quality outcomes in agreed timeframes.

Capacity to travel within Western and Far West NSW; hold a current driver’s licence.
Desirable
-
Previous experience in procuring and commissioning applications/IT systems used in healthcare services.

Qualifications in project management and/or experience in project management.

Experience working in the health, NGO or community services sectors
-
Experience in working with Aboriginal people, organisations and communities in view of planning and implementing services and achieving outcomes for Aboriginal people.
